Neurology:特发性正常压力脑积水的血管危险因素有哪些?

近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,研究人员旨在评估特发性正常压力脑积水(INPH)的全部血管危险因素(VRF),通过采用一个代表性的INPH大样本和以人群为基础的对照者来确定血管疾病在对INPH病理生理学的影响程度。

所有的INPH患者于2008年至2010年期间在瑞典接受了分流手术,研究者将这些患者与年龄和性别相匹配的人群为基础的对照者进行比较。纳入的这些患者年龄为60-85岁、无痴呆。研究人员通过使用血液样本、临床检查和标准化的调查问卷对10个最重要的VRFs、脑血管和外周血管疾病进行了前瞻性地评估。研究者评估的VRFs包括高血压、高脂血症、糖尿病、肥胖、社会心理因素、吸烟习惯、饮食、饮酒、心脏病和体育活动。

该研究共有176例INPH患者和368例对照者参加,多变量logistic回归分析显示,高脂血症(比值比为2.380;95%可信区间为1.434–3.950)、糖尿病(比值比为2.169;95%可信区间为1.195–3.938)、肥胖(比值比为5.428;95%可信区间为2.502–11.772)和社会心理因素(比值比为5.343;95%可信区间为3.219–8.868)与INPH存在独立的相关性。高血压、缺乏体力活动和脑血管及外周血管病也存在于INPH患者,而对照者往往为适度的酒精摄入和体力活动。人群归因危险度百分比为24%。

由此可见,INPH患者有更多的VRFs,以及在一般人群中缺乏保护性因素。几乎有25%的INPH患者可能被VRFs所解释,这表明INPH可能是血管性痴呆的一种亚型,有针对性改良VRFs的干预措施可能对INPH有有益的效应。
